Chronic care management is available to people with persistent symptoms from a chronic medical condition. The services we provide are similar to hospice care in that we offer treatment intended to make patients more comfortable and improve their quality of life.

Palliative in-home care for individuals with chronic medical conditions is typically covered under Medicaid, a program known as Chronic Care Management (CCM). The program is open to patients who have at least two chronic conditions that are projected by their physician to last at least 12 months. This could include conditions in which the patient receives curative care and conditions in which they opt to cease curative care, including Alzheimer's, heart disease, COPD, and diabetes.

The goals of CCM are threefold:

  • Reduce the occurrence of hospital readmissions by providing light medical care in the patient's home
  • Help patients better manage their health by mitigating the symptoms of their conditions and providing individualized education for each patient about healthier lifestyle choices
  • Improve each patient's overall quality of life. Reducing the frequency and intensity of flare-ups and treating the symptoms of their chronic conditions can make it easier for patients to care for themselves without assistance or even return to work

CCM services include regular check-ins, medication management, care coordination, education, and around-the-clock access to healthcare professionals.
